All PANTHER ATHLETES:
Boston Mtn Clinic will be doing athletic physicals in the old gym. Boys will be Wednesday, June 23rd and girls on Thursday, June 24th. We will begin at 8:30 a.m. on both mornings and end at 4:00 p.m. Up-to-date physicals are required to participate for all athletic teams, so please make arrangements to be there one of those days.
Boston Mtn. will require the new patient forms to be filled out prior to the physical even if this was completed last year. The forms will require a parent/guardian signature. Forms are attached below to print off and also available through the athlete's coach or the high school office.
FYI: Physicals done on campus will be free of charge; however, if you are unable to attend either day, you will be responsible for your own physical at your own expense.
