Flu shot forms, along with FERPA forms, will be sent home with students. Please have parents fill out BOTH FORMS and return to the nurse as soon as possible. She will look over forms BEFORE shot day and return to parents if incomplete. Parents MUST sign the Department of Health form in order to get a shot!
