*Social SecurityNumber
*BirthCertificate
*ImmunizationRecord
*HealthScreening
*Proof ofIncome
(W2, recent Tax form or current 30 days of payrollstubs)
Applications are currently available at Central Officeand Elementary Office.
You are welcome to pick up and return completedapplication along with documents needed before registration date.
